When Someone Dies Register a Death - Checklist

Registrar's Office

You need the medical certificate from doctor/hospital

Two doctors signatures required for cremation

You must be relative/person present at death/executor/solicitor

Death must be registered in district of occurrence

Within 5 days of certificate, extendable to 9 days

You may need to make an appointment

You need:

Person’s full names and maiden name if appropriate

Spouse’s full name & date of birth

Date & place of death

Date  & place of birth

Person’s occupation ( & spouse’s is applicable)

Details of pensions or benefits from public funds

Your details/qualification as informant

Medical   certificate signed by doctor

Decision of burial or cremation

Whether  deceased had lived there for more than 6 months

If person was under 75 industry/employment details

Registrar may ask for deceased’s NHS card

Obtain: (after checking details very carefully)

The registration of death certificate

Several copies (at least five) of the certificate about £3.50 each

A notification of death for the Dept of Social Security (form 344)

A green certificate for the funeral director

Certificates B & C for funeral & cremation
